Steve, your roof problem resembles exactly one we have here in the tropics on concrete roofs. We seal them with an elastomeric coating called VULKEM and when we get a bubble like you have it is because of moisture. Either still in the concrete or sweat dropped from a worker, or rain. Wow. One thing I wonder about : given how much this project is costing (your square foot cost is shockingly high), how can you choose things like doors and other fixtures?

I mean, you have a house that has been professionally designed to be unique, fit for an architecture catalog. It would be a shame to mess that up by choosing tacky interior fixtures.

If it were me, I'd probably hire a professional style consultant or something to get those inner details right. A few thousand bucks is nothing compared to the total cost of this baby.

Steve, is your heat and cooling from a geothermal or more standard Air:Air system? I would think that geothermal would be the way to go, but maybe the ground was too rocky. Also, you never mentioned installing the trenching and lines for a geo system, so I guess it is air:air?

You should look at either Xypex or Kryton to fully waterproof your slab--even good if normal shrinkage cracks occur as it will re-activate and seal them.
I am not a saleman for them--just know they both work well.
